Don't miss these important steps

  • Step 1: All students at Northwestern must submit the Admission Health Record and proof of immunizations as required by the state of Illinois.
  • Step 2: All new and returning full-time international students are required to purchase the Northwestern Aetna Student Plan; no exceptions or waivers will be granted.

Penalties for not fulfilling health requirements include a hold on subsequent registration and a non-refundable $100 late fee.

Admissions Health Record Form

  • All students need to fill out the Admission Health Record form and submit it by e-mail to Lisa Teel:
  • On page 1, when indicating your academic program, check “Undergraduate" and write “Exchange Student” in the blank area next to this. If you are staying at Northwestern more than 1 quarter you must either have a doctor complete page 2 of the form or you can submit a copy of your immunization records. If you are staying at Northwestern for only 1 quarter, completing page 2 (or submitting immunization records) is preferred but not mandatory.

Northwestern's Aetna Student Plan

  • The annual cost (premium) for an Aetna Plan is $3,449 for the 2014-15 academic year.
